  - Shift Requirements: 800-430 pm M-F, every 7th weekend rotation
  - Required Certifications to be Attached upon Submittal: Master's Degree in Social Work (MSW) and WI License
  - Minimum Experience: 1 yrs experience in social services and counseling in a hospital acute care setting with a diverse population
  - Charting Experience : epic
  - Unit Information : Med Surg and Critical Care Clusters- all inpatient units- Layered Model working in tandem with RN Team for Discharge planning
